Do I have to have an active PT Plan of Care at Concierge Physical Therapy to sign up for CPT Plus or CPT Premium?
Yes, all membership sign-ups require an active physical therapy plan of care (POC) at one of our clinic locations. If you have not been a CPT patient in the last 6 months you may require an initial evaluation.
Do I have to have an active PT Plan of Care at Concierge Physical Therapy to purchase session packages for Shockwave, Cupping, Golf, etc?
Nope! Plenty of previous patients continue to see us even after their plan of care is over because they love the benefits that recovery & wellness sessions provide.
Even if you do not have an active PT plan of care at one of our locations, you may still purchase and book cash-pay services such as Shockwave Therapy, Cupping, Dry Needling, Sports Massage, etc anytime!
How do I book appointments for my Membership benefits or Session Packages?
Appointments are not automatically booked when you purchase a membership or session package. You must contact the clinic(s) to book.

How do I cancel or pause my membership?
You may cancel your subscription at any time with 30 days' notice. You must contact us in person, via phone, or info@conciergephysicaltherapy.com to cancel.
Failure to provide 30 days' notice before cancellation will incur a cancellation fee of 1 month equivalent to your subscription.
Having surgery, traveling, or just want to take a break? You may skip/pause your subscription at any time, just ask!
I'm not sure I want to commit to a membership. Can I purchase individual sessions for Shockwave, the Recovery Room, etc?
Some therapies, such as Dry Needling and Cupping, are available to purchase and book in 1-session increments.
Shockwave therapy requires a 3-session minimum to allow for effective treatment and results. Each CPT patient receives (1) complimentary Shockwave Therapy session in their active Plan of Care.
Each CPT patient also receives (1) complimentary 40-minute Recovery Room session when they begin their active Plan of Care! Afterward, Individual Recovery Room bookings may be available upon request and as availability allows. Members are given first priority.
